Nursing Spiritual Assessment and Care Plan Obtain: Diagnosis, Prognosis, Future Medical Plan • The spiritual assessment for the Nursing Care Plan includes patient and/or family interview, review of medical chart, coordination with multidisciplinary team to identify the medical status and specific needs. • Medical conditions requiring urgent Pastoral Care: burns, trauma, amputations, obstetrical emergencies, fetal distress/death, cardiac arrest, code, palliative care, hospice, terminal illness, near drowning, cancer, traumatic brain injury, organ donation, all ICU admits, attempted suicide, end of life issues, food and hydration, removal of life support, DNR, advance directives. • Determine the religion, investigate unaffiliated or unknown status. If Catholic, request a Priest for the Anointing of the Sick, Reconciliation, and Eucharist. If other faith, call the minister of the patient’s religion. • For end of life issues consult Ethical and Religious Directives for Catholic Health Care Services (p. 111), in spiritual distress (p.59) request Pastoral Care. • In trauma, prolonged illness, impending death, assist the believing patient and family with prayer. Invite the patient and family to trust in God and in His merciful love for He is close to the sick. • For the dying, pray the Chaplet at the bedside and the prayer of commendation (p.81- 82). Encourage to seek forgiveness, God’s love and mercy. Remind that God is waiting to welcome him/her home. Offer image of Jesus, Mary, a holy card and a Rosary. • Baptize dying infants or fetuses by pouring water on the body and say: I baptize you in the name of the Father, and of the Son, and of the Holy Spirit. 62
